Programs

Sliding Fee Discounts

     Healthpromed provides its services to all patients regardless of their ability to pay.

     As a Federal Qualified Health Center, Healthpromed offers its patients tiered discounts for services provided at the center through the Tiered Discount Program. The discount scales depend on the income and the number of people under the family nucleus.

     On the day of your appointment, Healthpromed staff will assist you in determining if you are eligible for the Tiered Discount Program. Our team will ask you several questions related to your income and family composition to determine if you may be eligible for the program. If you are determined as a possible candidate for the Tiered Discount Program, you will be asked to bring the following documents:

Verification of Income and Family Composition

  • Income tax returns from last year (if you are married filing separately, you must bring both returns)
  • Last billing stub (for each family member)
  • Form SSA-1099 (Social Security Benefit Statement)
  • Form 1099-R (Annuities and/or Veterans Benefits)
  • Benefits received for the Nutrition Assistance Program (PAN)
  • Evidence of income from self-employment

Proof of Identity

  • Valid Driver’s License or Passport
  • Birth Certificates for each family member
  • Social Security card for each family member

Evidence of residence

  • Utility bill (eg. Aqueduct and Sewer Authority (AAA), Electric Power Authority (AEE)

Evidence of Health Insurance

  • Cards of Medical Plans, Reform, Medicare, etc. if any

Community Relations Program

Develops and implements programs and activities that result in a positive impact for the community. Assuming common objectives and developing activities from an interdisciplinary approach, with emphasis on the individual, family and community. Promoting prevention and health education, joining efforts and promoting intersectoral relationships between the organization and groups such as: community leaders, government agencies, non-profit organizations, private, educational institutions, among others. In addition, through the Community Outreach and Discount Program, accessibility to primary health services is guaranteed to people without health insurance or limited coverage regardless of their ability to pay.

Outreach and Enrollment Program

  • Identify participants without medical insurance
  • Facilitate consumer enrollment through Medicaid, Medicare, or CHIP
  • Provide guidance on the eligibility criteria for the Tiered Discount Program

School Health Program

The School Health Program provides services to students between the ages of 11 to 16 from the Federico Asenjo and Ernesto Ramos Antonini Specialized schools. Both located in Barrio Obrero, Santurce. 

Through the services of pediatrics, nursing, behavioral health, nutrition and health education, they contribute to the comprehensive health of the student population through health promotion and prevention. 

By maintaining a shared vision with school staff, academic achievement is maximized by reducing the social determinants of health, thus achieving student success. 

A healthy school climate and well-being of family and staff is maintained, providing support in a safe, convenient, and accessible environment.

As part of the services, extracurricular activities are offered, such as: the summer camp “Muévete con Sabor”. Encouraging adolescents to enjoy a healthy lifestyle by increasing physical activity, healthy eating habits and entrepreneurship. In addition, the Health Club encourages the development of leaders where they can explore their interests, connect with their peers, and inspire their peers, families, and communities to practice a healthy lifestyle.

Programa de inclusión LGBTT+

In our LGBTT + inclusion program, mechanisms are promoted that guarantee medical care without discrimination to LGBTT + people, through the creation of a protocol of general observance in the provision of HPM medical services. Specific care criteria are established in the medical care of lesbian and bisexual women, gay and bisexual men, transgender people and intersex people; through the creation of specific reference guides for the care of each of these populations. We promote a culture of respect for the human rights of LGBTT + people in all the personnel working in HealthproMed. Contributing to effective access to quality health services, by establishing guidelines for action that avoid discrimination for reasons of gender identity and/or expression, variation in sexual differentiation and orientation.
